There's no gamepad support. You'll have to use JoyToKey or a similar joypad-mapping utility. It can do a pretty good job with KOTOR if you configure it right.

forget Joy2key, its not the best. instead use:

and instead of a USB joypad for your pc, get the xbox-USB adaptor for your (maybe) xbox controller.

you can map the mouse movement to the right stick and if you adjust the sensitiveity properly, its not a big undertaking to do it, you will have some wonderful control.
