Requesting sick leave for cancer treatment?

Long story short, my mom recently diagnosis with cancer and require chemo treatment for possibility 3 months. She have a part time job at a office and she doesn't want to resign yet but to request 3 month sick leave for the treatment. She ask me to write a letter to the human resources department and I am not sure how to do that. Is there any kind of special form she need or something? Any advice is appreciated.

Unless your local city of state has special rules you need to reference, just keep it straightforward, factual, and brief. She really needs to talk to her boss directly though and ask them. They may have a form to give to her doctor.

My suggestion is to be as up front and direct with your mom’s company as possible. You are going to need to go through their HR department and possibly her direct supervisor(s) depending how the company conducts itself.

Does your mom know who you need to contact at the company? If not, I think your first step will be to look them up online and get an idea of how to reach out to them. Depending on the size of the company, that could either be a simple task or a complicated one.

Just phone them and talk human to human. If you need to write an official letter then they'll tell you so. No one's going to argue with you bro.

First- the boss doesn’t need to know what the medical issue is and shouldn’t ask you. Just ask to talk to HR. If your mom has met the work requirements for it (sounds like she should have), she’s entitled to FMLA. HR may require a doctor’s note saying she’s under their care but that’s about it as far as I know. It’s been a while since I had an FMLA refresher but I was told to just send my employees to HR and only give them words of encouragement and support, no questions about details unless they volunteer information on their condition/want to share.

You should also see what your state offers

go to reddit cancer/ Or a state specific forums and ask around. People like to help, the problem is finding those people and sifting out bad info.

i wish you the best. My mom had breast cancer last year and it is ‘gone’ now, so stay positive.
