Here is something I whipped up over the last few days:

The dimensions should be approximately correct, though I could only estimate them using the pr-shots.

Some tips:
- Use an exacto-knive.
- You should go along the fold-lines with your knive, but only apply a small amount of pressure.
This will help making clean folds (maybe you should try this technique on a blank piece of paper first).
- Use a toothpick to spread the glue, only a thin layer is needed.
- Use paper with a weight of 100-120 g/qm.
