Sending Back PS Underground Disc- What's the Address?

I called Sony the other day about sending my faulty Holiday disc back in for a free game, but I don't think I heard the address correctly. Does anybody have it?

BTW do you need to send in your ps2 serial numbers?

You need to send your name, address, phone number and the name of the game you want. Of course, you'll also have to send the demo as well.
 
Since you've posted the address is there any point in calling them, or do they need to hear from you on the phone before accepting it in the mail?

Yes, you need to call. They check how you recieved the demo (they checked it and found out i got it through Underground) and they type something down (case number prolly). Seems like maybe too many people have taken advantage of this?

Also, they ask what your PS2's model number is. Course, I just told them that my network adapter is in place and they said they didn't need it anymore.

As an apology to gamers who lost their PlayStation 2 save data to a buggy Viewtiful Joe 2 demo on the recent winter PlayStation Underground demo disc, Sony is offering a free game to victims of the glitch.

Wounded consumers should contact Sony Computer Entertainment America support at 1-800-345-7669, with their PS2 serial numbers handy. Sony will then supply an address at which to exchange the defective demo disc.

The SCEA-published games on offer include the following:

Gamers may also want to note that while the most recent Official U.S. PlayStation Magazine demo disc shares some content with the defective Underground disc, it does not contain the glitch in question and may be played on your PS2 without fear of any ill effects.
 
I've read a lot of conflicting reports on the games you get for free.

When I called it was all of that list posted above plus more sport titles and Jak 2. Anyone else want to share what other games are offered by phone first hand (not what these websites are posting)?
I've heard Ratchet and Clank, Dark Cloud 1/2 and Drakken offered on another post on this forum but are these confirmed?

I think they changed the game list like a couple days after the offer was announced. They must be running out of copies or something
