What's extending the games is all of the flopping. Referees generally don't extend time for things that happen during the normal run of play line goal kicks, throw ins etc. Stoppage time is often called injury time as a reference. There have been a ton of serious injuries but also flops which make games last forever. Don't blame anyone other than the players.
No, it would still require an unlikely 5 goal swing at least.
Ghana would have to make up a 5 goal difference between it and Germany for that to happen. If the US and Ghana win, chances are Germany is still advancing.
