The majority of billing issues that I dealt with were customers that attempted to use PayPal and something went wrong in the handshake process between them and BBY, cancelling their order. And 99% of the time, we didn't have any tools to help them, other than ask Paypal and attempt to reorder. Credit cards didn't have anywhere near as many issues and we had some tools to help with those problems.
That's just my (and my coworkers) experience though. Take it for what it's worth.

I did, it took me about four tries.
One per costumer+Prime only helps a bunch. I doubt there are many peeps paying for prime on multiple accounts.
