The Neuromatrix effectuation and schemas of Phantom Pain are all presupposed by probative concepts and attestations. (Reingard., 1987).
Here's what I think is going on:
It doesn't seem to be there when he's brought into the hospital in the Ground Zeroes section. His face is bloodied, but there's no shrapnel sticking out of his forehead. When he wakes up in the Phantom Pain section, you can see a protuberance under his bandages on the right side of his forehead. Later, in his Diamond Dogs getup, he has a full grown rocky horn growing out of his head.
I don't know if he grew it while he was in a coma or what, but I think it's more than just shrapnel.
Maybe it's from the car crash?
I'm sure it's been mentioned but I think Diamond Dogs is in reference to blood diamonds and the horn signifies BB becoming a villain.
No clue who they're referring to with "the woman" and "him" though (assuming Ishmael is not imaginary I'd guess it's him as in Moby Dick you see everything through his eyes)

Perhaps the assault on Outer Haven by scarface/Zero didn't go so hot, and he was the third party injured in the hospital. Hence the raid.

These past few years, Konami seems to slowly be "forgetting" about that entry though. Several of their recent books purporting to cover the entire saga skip it entirely.Portable ops is canon. It is the direct sequel to MGS3.

What if this game is about Solidus? Mother Base is destroyed and Big Boss goes into a coma, but wakes up and walks out of the hospital a short while later. The Patriots use this opportunity to kidnap Solidus and use a young Psycho Mantis to "implant" Solidus with Big Boss's memories in an attempt to make them the same person. The whole thing goes wrong and Solidus emerges from the induced coma hating the Patriots, getting to know Ocelot (him arriving in the trailer and rescuing him on horseback could be their first meeting), and later orchestrating Shadow Moses to get back at them.
