Only Act 1 (of 3) has been available during Early Access.

I'd say that easily 70% or more of the actual game will be completely new, aside from any changes or expansions made to Act 1 for the retail build.

Oh yes, the soundtrack for this game is great.

I really like how unnasuming the tracks in the game start, then they devolve into these rich and poweful melodies as they go along

It was one of the highlights of their last panel, where they've shown this trailer for the first time.
Both that and the Dark urge origin, as well as some other evil pathes you can do (which are also on the trailer).
