There are a ton of great horror games. I'm not talking about those. Resident Evil 4 is an amazing game. In no way is it a "Halloween" game for the points of this discussion.

I'm talking about jack-o-lanterns and October 31st.

What are the best Halloween games specifically?

I've noticed rom hacks seem to be the big winner in this space.

Costume Quest is probably the best Halloween themed game.

There are some that add decorations for Halloween but aren't specifically Halloween themed.

Medievil during Ps1 era was the best for me. But this generation it will always be Bloodborne. To bad there is no sequel or remake yet from Sony for BB.

This is a bit of an odd recommendation, and it's still way overpriced, but Epic Mickey Rebrushed has plenty of twisted, creepy and messed stuff happening that could be considered Halloween-y. And it's actually one of the better remasters out there IMO.

Sheeeeeeit I might just go reinstall. Forgot how pretty it was. And I only need a couple more trophs for the Platty.

but only the PSP remake is actually a decent game... because they actually changed the gameplay in that one and made it more similar to the second game.

I had an N64 instead of a PS1, and never played them, but always thought they were cool. It was the first time I played it, and I really wish they'd redo the others, but I'm guessing it didn't sell well. They should at least release it on PC.

The creators of those games, Doublefine just released another otherworldly game wth another otherworldly atomosphere.
It's kind of creepy.

If you can find it somewhere for $30 or less you should give it a shot

There's a game that goes balls to the wall with the early 80's style. There's a virtual 3D arcade machine with intricate bezel and side art. A cellar to explore which a bunch of stuff and some fictional 30's comics the game is based on. Lots of Popeye & Peter Pack-rat influence here.
The theme of the game even sorts fits the scary halloween theme. Your last remaining innocence will wither away looking at it.

I'm playing this currently and it seems like a spiritual successor to Ghosts and Goblins. It is easier though but respects the arcade mentality of routing, scoring and has a time limit.

It's the best classic ghost and goblins clone. Battle Princess Madelyn also tries, but fails (it's not very good).
BPM devs did a Burgertime clone which is pretty decent and fits the Halloween theme.
