I saw some weird void in the place right after the cathedral safe zone thing, it kept popping up after i got some armor (top hat and hunter gear) from a body there. I went into one and it just picked me up and said "frenzy" or something, almost killed me but I had a little health left over, anyone know what is going on there?

Using a broken weapon explains the extreme difficulty you're having, that's for sure. Not sure if armor can break as well, but I'd check that too. Use the blood dew you find (item description is that it gives you blood) and repair your gear.

Yes, if you die and then die again without getting your "souls" (blood echoes) back then they're gone forever.
