Developers that release nothing but hits

Intelligent Systems is the closest I can think of, but even then, not all their stuff sells like hotcakes regardless of how awesome it is. Especially factoring in Japanese sales, where nobody really buys anything from the Wars series.
That's alright. US Advance Wars sales make up for it.

..again, has Game Freak had anything other than a hit in the past 10 years? Have they had failures before that point?

I think Nintendo EAD (which Nintendo R&D1 was absorbed into last January) probably has the best track record as far as releasing the widest selection of genres/IPs and still having them sell and score highly.

Intelligent Systems (a satellite developer of Nintendo Co., Ltd) might have released a higher percentage of great games to the hardcore gamer, but they pretty much have worked on the same game mechanics for the past 2 decades.

* Fire Emblem, Wars, and Panel De Pon.

Which is completely opposite of Nintendo EAD, who even in sequels, has proven to completely change the game mechanic.

Then again, my personal choices would be for the developer with the highest percentage of quality and commercially successful software

1. Nintendo EAD
2. Blizzard
3. Rare
4. Team Ninja of Tecmo (Just squeezed in because of Ninja Gaiden)
5. Intelligent Systems

Yeah that's true actually.. Both EAD and R&D1 were far more experimental. IntSys did make Panel do Pon though and that's the only puzzle game that surpasses the original Tetris in my mind.

Nearly all EAD's games are million sellers actually. I'd go so far as to say the average EAD game outsells anyone else's average.

Even the Shenmue series would be considered a flop given the production budget. I like the games myself, but super hits they were not.
 
Insomniac - First 3 Spyro games and Ratchet 1-2-3, all GH or multi-million sellers (I guess you can count Disruptor too since it was a good game, just didn't have the marketing $ at the time.)

Blizzard - Warcraft, Starcraft, Diablo all enormous hits.

Bungie - Pathways into Darkness, Marathon, Myth series, Halo. Most of the Bungie games are pretty damn good and have sold well considering they are mainly console - exclusive / mac-exclusive titles.

Bioware - Baldur's Gate, MDK, KOTOR
