There is a specific company that makes all of these steel books. I had the same problem with the Witcher 3 CE and they replaced it free of charge. Go to Steelbook.com. Their customer service rep got back to me in 24 hours.
Looks like that site is for European countries.
So mine steelbook was broken too. I emailed Bethsoft about it.
Here's my steelbook, which I included with the support ticket, along with my proof of purchase.
And their response to me explaining that it was broken but in the package as if it were broken during assembly:
Amazon offered a 10% refund and that's it.
So we're fucked, basically, unless a news site picks this up and Beth is forced to fix it. Steelbooks aren't like the pipboy, they're constantly manufactured.

Well of course not, and neither can amazon. I don't want to exchange the thing, I just want a new steelbook which is handled by a completely different third party company.
Amazon offered me $60 in credit. Guess I'll take it.
I feel like I got the short end of the stick just now ._., the 10% back I got is only $12.

Said I wanted the extra $60 I paid back, or a replacement.
They said they couldn't replace it, but agreed to credit me $60.
That's BS that you got $12.
I think you did man. I got $25.50 refunded for only having the clips on my steelbook broken.

Sorry if I got the URL wrong, it's on the steel book itself. Look on the inside toward the bottom. The steelbook company will replace them. They replaced my Witcher 3 steelbook without charge and never asked for the original back. They seem to keep extra on hand for when these games launch.

I was in the same position up until 20 minutes ago when my order updated. Now it says the order has shipped, there is a tracking number, and it is set to arrive tomorrow by 8.
Really hope it isn't damaged...

Bethesda will not replace the steelbooks, they're made from a completely different company. Reach out to them.

So we can't even get the app to work with the game on PC. It just wont find any games, we turned it on in game, we gave permission to the firewall when asked, and still nothing.
Did you restart the game?

Yep. And rebooted PC, etc, nothing works. We got a comcast wifi router, dunno tried some port forwarding also that we saw on reddit, did nothing for us.
Sent them an email explaining, lets hope it goes well.

I had to change cards due to some fraudulent charges, but BB sent me an email last week letting me know to update my payment choice, which I did. Now I am the happy owner of the Pip Boy edition.
I have to say that it looks much more impressive in person, even unopened. I don't plan to open mine so I won't be worrying over any QC issues
Used the contact us link on steelbook.com, it's to their parent company that owns part of the rights to steelbook.

PBE arrived today from Amazon, but whoever packed it didn't use enough material and it was just rolling around in the box. The case was a little marked up unfortunately.
Emailed Amazon, told them what happened, and they issued me a full refund and told me to keep the game.

Circled it. Don't press the light on top either. The light under the screen is the power button.
Yeah, I pushed that light thing. It springs back up, but only to the point where it's flush with the black ring around it :/

I'm trying to get it back up by sticking tape or something to the top of it and pulling, but nothing is sticky enough.

Okay, I just pulled that clear button/light on the top back out, and here's how I did it (it's a weird fix, so your mileage may vary);
Stuck a piece of scotch tape to the top of the button. With the tape still on it, I pushed the button down into its cavity and then quickly pulled the tape up. It took a couple tries, but on the third or fourth try it pulled right out with the tape.

Edit: Literally four minutes after I posted this my cries were heard. As of 12:26 am the package is "out for delivery". What I would give for the UPS door man to show up at even 2 AM with my game.
My Amazon purchase is lost in the wild somewhere apparently.
Amazon's tracking says it was picked up from their warehouse at 9:55 PM Monday night. UPS tracking says a shipping label was created and no further action was taken. UPS has no record of the game being scanned at any point in time. Amazon says it no longer has my game on hand.
They say it does happen time to time where things completely get past scans and don't show up on tracking until they are delivered, and I'm pretty sure I've had something similar happen where it scans in somewhere far away then scans as delivered with nothing in between, but I've NEVER had anything like this where it is just completely unaccounted for by any party.
I called Amazon twice, UPS twice, and neither have any idea what to tell me other than wait until tomorrow and hope it pulls up on a delivery truck into a UPS center. Unbelievable. If it ends up lost and all they offer is a refund and a month of Prime I am going to be getting in someone's ass.

Got mine in the mail today from Best Buy and just finished giving it a test run.
Everything looks great and seems to work perfectly. I mean, as well as a hunk of foam and plastic can work, but it looks great. I'm glad I read about that black button here, and I definitely don't recommend fiddling with dials or anything. It all feels pretty fragile.
I got my iPhone 5 in. Eventually. Those foam inserts are snug.
I also just noticed a little tear in the padding inside, but I think I did that while trying to get it off with the band pulled too tight. Still looks pristine on the outside at least.
It was a poorly hidden Christmas gift from my girlfriend that I knew she had bought for me. What I didn't expect was that, in fact, she hid it so poorly that she had it shipped to my house and told me to open it when I inquired about the package. So Merry Christmas, everybody. I hope everyone still waiting gets theirs in good shape.
I also feel extra terrible because I'm halfway through playing New Vegas and I'm not ready to start this one just yet.
