Some years ago I traveled to Japan and lived at an old Japanese master of Aikido for 3 months. Woke up every morning at 4:45 to pull up weed in the garden for 2 hours, then practiced for 3 hours in 40 degree celsius (104 F). Then rested for a couple of hours before further training on my own for a couple of hours, and then again official dojo training in the evening for 2 hours. Day in day out with only monday to restitute.
One weekend we went to a high profile seminar/camp. The night before I had been drinking together with my sensei, and I was tired as hell. For one of the lessons at this seminar we had to train with the same partner the whole time. My partner was some crazily good young(around 30) Japanese guy who just had to prove how much better he was than me (apparently a no-good-foreigner, who thinks he was something because he got a black belt), and more or less beat me up for an entire hour, deliberately throwing me on the wooden floor several times.

That I never flinched once, and got up everytime he had thrown me or had me in a lock eventhough I wanted to stay on the ground/had to puke. That I thanked him after class without a single negative feeling in my body is my personal greatest accomplishement.
 
In two years I transformed myself from a anxiety ridden, depressed, 251 pound man with zero prospects into a 174 pound graphic design major with a beautiful girlfriend.
 
Really struggling to think of anything.

I did manage to graduate from university with a 2:1, but if whoever marked my dissertation had been more careful he should have failed me as I fucked up massively on the day it was due to be handed in, accidentally omitting lots of crucial data from my final report, so don't really feel especially proud of that.

I've err, been addicted to both pills and booze but got myself off both, but not really sure if I'm any happier for it.
 
My diploma in Automotive engineering (at every appointment people can't believe that I managed to go from the worst educational level in Germany to this (it's called Hauptschule in Germany)).
 
Right now, probably an award I won at some high journalism convention thing for writing a review of an art gallery. There was a ton of kids cramming who knows what from AP books outside the room we wrote in. I just kind of BS'd it and ended up getting the second highest award offered. I mentioned Hideo Komjia in my intro, which is extremely embarrassing looking back.

In about a year finally getting my bachelors will be my biggest achievement. I had a rough start, basically wasted a year fucking around at a community college failing classes, almost got kicked out of my house, moved to a different JC, did alright for a year, started slacking again, and then took 24 units while working my last semester to graduate on time. Since then I've been a lot more motivated, and hopefully after I get my bachelors that achievement will quickly be replaced by getting accepted into a decent Masters program.

Really I haven't done a whole lot to be proud of. I've been getting pretty antsy lately, can't wait to be done school and start a career.
