You need to actually go to your local court and have it done by a judge. Check with their main number, the operator should be able to tell you exactly what you need for your district. All those websites offer is a little help in acquiring and filling out forms you could do by yourself, they can't legally do it for you.

you can usually do it at the secretary of states office for a nominal fee. ITS CHEAP... LIKE BEER!
