It's not a simple fix because it's not a simple problem.

Based on what I have seen his HDD is either corrupted or his HDD is defective.

Since the OP and a few other people have this exact same problem I am leaning towards the HDD being defective and their's a batch of them apparently.

If this is the only problem then we can at least buy a reliable hdd to replace it. But it might also be a problem with the ps4 which means you're screwed either way.

Channel subscription you need to chat, does user activate that, I dont go to twitch often, did he do this because getting lots of views?

It's to slow down the chat so the Twitch admin can talk to him. Also it was getting super crazy, even for chat monsters.

Maybe the OP should try to force the PS4 into recovery like this streamer just did.

Boot without the HDD and it throws out an error because it can't start. Then put in the new HDD and hopefully it will go straight into safe mode and follow through with the system check.

Absolutely, but I think this is a problem with the HDD because whenever he tries to write to the HDD (Installing, downloading etc.) his PS4 dies.
