It's almost certainly just a metaphor. Kojima was just showing that Big Boss is metaphorically turning into Skullface because his quest for vengeance is turning him into a horrible monster like Skullface.

There is a scene in the E3 Kojima trailer of the two of them riding in a jeep together.

tbf though, no one besides BB seems to aknowledge skullface's presence.
Elegia trailer even suggests skullface being some kind of hallucination in a few scenes.

If it weren't for this trailer, I would think there's going to be some "Fight Club" twist somewhere, either right from the start with Ishmael / Big Boss or Skullface / Big Boss.
Now, this trailer hinting at this though, is exactly why i don't think this is the case anymore.

well that Metal Gear is the most Japanese looking Metal Gear I have ever seen and that transition from MGS3 - PW - GZ - TPP - MG2 - Skullface is not to be taken literally I think. Skullface exists already so I assume its a metaphorical transition to show that Skullface has influenced Big Boss' mindset into the villain we see in MG and MG2.
