You show them a demo/reel with some interactivity examples, you tell them how is going to be and they rate it. At least that's how is done over here AFAIK.

Basically, the developer shows the ESRB the worst stuff they've got...Anything violent, obscene, sexual etc. That's why Rockstar is in trouble with San Andreas, because they didn't show the ESRB the Hot Coffee material.
