I should note that all the bans that went through were looked over, generated, and discussed by 3-5 mods. We do take fairness very seriously. The number actually varies because the process can take hours when deciding a direction we want to go with moderation of an emerging console war.
Hive mind posting is when people direct blanket insults against "GAF", "those people", or any other such variation where someone wants to argue with a posters, but instead of addressing them directly, they just make a blanket attack in their direction.
EviLore put the policy into place about 1-2 years ago and went on a campaign warning people we would start banning for it. The reason for this is that it isn't really a fruitful discussion when someone attacks an unstated group of people, as no one can respond to that claim and defend themselves against it. Instead, it is just a way to insult people without trying to address their point.
I believe the offense is stated in the FAQ, at least the version we put out in 2011.
