The thing is that not only do we have the werewolves to deal with, but we know there's a serial killer in play. At least, I'm pretty sure we do from this line:
They should be able to kill once per night, right?
So then, I would think under normal circumstances, at least two people should have died. Assuming 2 kill orders were sent, that would mean 2 doctors managed to save both potential victims, which is highly unlikely.
Assuming 1 kill order was sent, that means 1 doctor managed to save 1 victim, while the other killing faction didn't send any kill order... which is kinda weird. Sparing someone so they don't have to die before day 1 starts?
No kill order being sent would be extremely strange, and the only thing I can think of is that both factions decided to not kill anyone because this was a night start; a sign of mercy? Weird thing, if so.
I also think this means we don't have a vigilante in play, because 3 different killing capable "factions" resulting in no one dying seems impossible.
I can hear Palmer wincing from here. Don't read too much into fluff. Palmer is the anti fluff. He's way fun at parties.
Vigilante could have chose not to use his kill last night since There's nothing to go on. Doctor could be possible but not definitive.
