If Kojima challenges someone's gender/sexuality or thoughts on gender/sexuality, I'll be left in awe. Mind you someone finding a transgender person attractive isn't shocking in any way, and the idea of someone being transgendered isn't shocking in of itself, people should welcome and support someone for who they are mentally, emotionally and spiritually.
However, look at the internet and gaming culture in general. Venomous attitudes towards gay/lesbian people, towards women, towards all transgendered people. Getting bigoted or ignorant people to question the idea of gender and sexuality in a mainstream game would be a magnificent feat.
The idea of phantom pain extending to self identity and gender? Fucking brilliant.

Secondly, I think it would be quite insulting to the actress if the character she was based off was in reality a man - not trying to be insult anyone, but it would imply that she looked manly. I don't think Kojima would do this, it'd be a rather low punch to the face cap actors he hired and could hurt his reputation.

Kojima based Quiet off a real life model/actress or whatever the fuck she is. Meaning they paid her money for her looks. I doubt Kojima went out of his way to look for a woman with similar facial structure as Chico, so that crosses out the similar facial theory.

I believe Kojima stated that they couldn't find an ideal actor for Chico's appearance, so they combined the appearance of different (kids?) to get the look they wanted. This means they could've went out of their way to alter Chico based on the actress and character they came up with, rather than the other way around.time to debunk this theory;

Also, Snake and Ocelot look very different from their actors, so it's definitely possible for them to make visual changes to characters while maintaining facial mapping.Quiet actually does look "manlier" than her IMO.

Watching the Red Band trailer after playing GZ is cool. Nice to have an understanding of the mechanics and what is going on during the 'gameplay' parts.
Why is the game so far away in time? Judging from Ground Zeroes and the Phantom Pain trailers (which is 10 months old) you'd think they've completed quite a big part of the game.
