Oh my lord. I think you're putting way too much faith into pure coincidence. This game has been in development for 5 years, has tons upon tons of missions, something like him putting credits into every mission isn't some sort of prophetic inclusion. It's merely a design decision trying to emulate tv.
I asked Kojima what he'd like to cut from his life, during an interview for Edge magazine in 2013, who spoke openly of his plans beyond games. "Tough question. Maybe Konami (laughs). I'm 50 this year, and while I love game development, it's a time in my life where I'd like to explore other things. I love games, but I don't necessarily like the management of the business aspect of working for a company. There are many things I'd like to do: for example, maybe take a year off and write a novel. I've also had offers to work on various movies and make movies, so those are things I'd like to explore at some point. If I could, I think that would be a great change in my life."
This is just speculation on my part, but I imagine that Konami's US employees aren't too happy with what's happening. The company's "new course" pretty much spells downsizing for the western subsidiaries.

Honestly what do you think is more likely, design design based on the above, probably one of the earliest design decisions made for the game or Kojima can predict five years into the future on how Konami will be petty with his name?

Nah. Metal Gear Solid 6 will be massive. New blood, new ideas and coming of the massive marketing budget that V is getting they're turning MGS into a bigger and more relevant franchise this time. They'll make 6 and if that bombs Konami is probably done on consoles.

It's more melodramatic than the usual company fallout, but it's pretty par for the course so far when it comes to company rumblings. "A Hideo Kojima Game" has been a thing on KCEJ/Kojima Productons games he directed for a long time.
There no way that fans wouldn't notice if it's removed.
