I'm pretty sure that if it's set-up properly, there is no way for Phoenix to escape. At the very least, Teleport won't work, as the move has a bit of start-up; the set-up is designed to avoid the blast from the Dark Phoenix transformation then catch her right as the invincibility ends.

Champ's loss against Raccoon was definitely more on his part, as he underestimated the length of time the trap remained active and/or misjudged the spacing.
