These days I'm replaying silent hill book of memories a little bit and these are the biggest differences with the patch:
pre-patch:
-when you die you lose all unsaved progress and reappear at the beginning of the level or in the save room after reloading the level
-poison traps leave who passes over it(player or enemy) with 1 hp for a certain number of seconds
post-patch:
-when you die you do not lose any progress and reappear at beginning of the level or in the save room(without loadings), but you are forced to retrieve keys, medkits, repair kits and bullets from the place where you died(similar to demon's Souls)
-poison traps thankfully are not changed much, who passes over it stays with more energy than before and for less time.
In words they do not seem big changes, but the game is now much more straightforward and forgiving, before the patch death used to mean redoing everything after a save(or after the beginning of the level), forcing the player to play with great caution, cunning and planning (a bit as I did with demon's Souls), since in each dungeon there's only one save point (and many dungeons are really big) and you don't even know where it is.
I guess that many will now find the game more accessible and perhaps better, but i have to say that i liked its being extremely punitive and unforgiving.
