Yeah they shot the pilot with her and Jennifer Ehle as Cat. As far as I know they replaced her with Clarke simply because they thought she would be a better Dany.

When they filmed the pilot, Sean Bean had short hair, so he wore a wig and because of Fairley replacing Ehle, they had to refilm his scenes with Cat in the first episode and there is a significant difference from the wig and Bean's real hair.

And yes, there is no episode this week
Any way to see the original pilot?

I believe Tamzin Merchant (the original Dany) was nude in the Tudors so you guys can check that out if you want to compare asses with Emilia Clarke.

I think they just didn't think Merchant "popped" as Dany in the pilot. In the bonus content on the first season they said they had actually flown the finalists for Dany out to LA to perform for HBO execs. They really wanted to get it right. Also, GRRM (who wrote some books you might have heard of) preferred Merchant and was pissed they replaced her back when it happened.

I can't believe Ned was alive this whole time!

Well, not really. Ned's soul resides in the giant mech that Jon found. I didn't care for the book change though. I know travel convenience is needed in tv, but the book mech couldn't fly. I figure the other Stark kids will get their mechs next episode, and they'll combine into Voltron just in time to giga-beam Kings Landing.
 
Damn looking at little Bran and Arya. What are they going to do when they get too old to be believable. Bran already sounds like a teenager, I predict by the next season if he's still alive or whatever, we'll start to see beard stubble.

Same goes for Arya. Supposed to be a little girl with that small sword, now she looks as tall as Jon Snow lol.

Epilogue of tonight's episode:

"Hodor", murmured Hodor as he slumped into his seat. His legs were aching from climbing these steps so many times. He thought back on his betrayal. Everyone had thought him a simple stable boy. Little did they know that Hodor was playing them all for fools. No one had expected him to smite the Wolves and the Lions. No one had expected a simpleton to tame the Dragon.

"Your Grace?" asked the Hand. "How shall we deal with the threat of the Hot Pie? With the White Walkers coming to his cause, they hold superiority on both land and sea. How can we possibly defeat them?". Hodor looked at his Hand. "Hodor. Hodor? Hodor Hodor Hodor! Hodor", snapped Hodor. "Yes, your Grace." Stannis scurried out. Only the King and the Lord Commander of his King's Guard remained in the hall. "Hodor". "Corn?", replied Mormont's raven. Hodor look at him deeply. "Hodor", said Hodor gloomily. Hodor felt fear run through him. How could he possibly defeat the Hot Pie in single combat? The Great Other was no one Hodor wished to face, but he could not meekly surrender the Kingdom of Men.
