Do you just play at a very leisurely pace, or does this game actually have 200 hours of content? I 'finished' my first runthrough in 43 hours albeit I didn't do the Kerry line and stopped halfway through the River line. I plan on getting these on my second playthrough which I have already started. I don't feel like I missed a ton a "good' content, but I guess I can't be certain.
I wouldnt say I play leisurely, but I do like doing things outside of quests and there were a few other factors:
1.) My character is a stealth hacker...especially early on (i love trying to do areas Im not supposed to be yet as an underpowered entity) you spend waaay more time on a quest if you go in silent with the goal not to be seen and to clean up all bodies etc etc. Often you have to reload and start again as you fail. If you go in guns a blazing it can take an order of magnitude less (i.e.ten times less).
2.) I love to search the map outside quests and find unique things on my own. I found the Roy Batty easter egg on my own just by checking doors for example and found a way to get on the monorail. Later, after I thought the monorail was blocked off by artifacts, I learned you could double jump or duck under the artifacts and I made it around the entire map just about. This took a whole evening or 4 hours by itself. I found the whole area the parade takes place in early right after coming off the monorail and it was amazing fun to clean it out...the grand prize was the legendary pancea crafting spec. Likely one of the best random loots in the game. That took another 4 hours as I searched every inch of the walkways and fell off the buildings trying to jump the levels to get around without knowing how to do it by going inside the buildings.
3.) I caused the worst traffic jam possible after following Judy at close range in her van to see where she would go after a quest (she goes on random not programmed loops which is the same for all vehicles.) The traffic jam involved 20 -30 cars eventually as the ones that line up in a row will move if you bash the car in front of them out of the wya or have it explode.
4.) Special gear. I learned of Skippy early but did not want to be spoiled of his location. I did spoil the general area on the map at least so I had a chance, but it took time to go on randon searches throught he area, up high down low etc.. I also wanted the full netrunner (legendary) set and searched a lot for the 3 other pieces besides the jacket which you get in a quest. I got general areas from you tube but each area is on 3 separate sides of the map. I finally found most of what I was looking for and most random loot in the game (although Im sure I stil have a couple left cool finds left). These arent asssociated with quests.... there is tonnes of random high quality loot in this game.
5.) I searched underwater for a few hours ...there were only a few cool finds related to that endeavour...but it ate a lot of time.
6.) I wanted to find the location of Dexter Deshauns body on my own...took a few hours.
7.) Found Kerry's house early, as well as that Araska hillside mansion, cleaned it out.
8.) tried for hours to hop on the AV's landing pad down by the water. I eventaully used a similar strategy to someone elseon yu tube and jsut watched him do each one. It was damn hard to get on though and he must have spent 10 hours to do all 4 AV's....10 hours likely. I spend at least 4 unsuccessfully.
9.) I searched all boats I found inthe game, there are two others in addition to the one in the late game quest where you get johnny's car. One of them is guarded heavily and I was low level so had to swim and thigs to stay undetected.
10.) Did a tonne of using the on board camera, trying different poses etc.
Im missing a few things that were cool, also reloaded saves a couple times, like at the begniing I missed too much I felt at arasaka tower but no one can say I didnt try to get my moneys worth out of the game....I havent even started past the point of no return yet and will likley reload to get all endings.
