Generally from what I've seen new items become available
to buy after a boss...this was an exception in that it dropped after the boss using it was killed.
Boss seems like it might be the new
Old Monk PVP type boss...not confirmed until the servers come up obviously

Happens to the best of us.
As far as where to go.. I guess just try and memorize the world and try to search every path you can. Opening shortcuts helps traverse the world quicker.
You can't level up until you encounter your first boss (don't have to beat it). After that you can level up in the dream place.

The game's world is interwoven all around itself so its normal to feel like there are a huge amount of ways to go. The good thing about this though it there's shortcuts through the levels. Try looking for a shortcut that on the way to the First Boss. Hint:
On the Bridge
Checkpoints will be few and far between. So it really helps to find the shortcuts throughout the levels, or try to memorize the fastest path through. The game wont point you where to go. You decide that all on your own from obtaining and using items that let you progress if you come to a dead end.
If I remember correctly you can level up in the Hub after
meeting the first boss once.

Enemies that kill you (or are near you when you die) feed on your blood :3
you unlock that by killing a boss?
Like others mentioned sometimes an enemy will pick it up and you'll need to kill that enemy. It doesn't always seem obvious which one though :/
The boss
is wearing it and seems to lead the player down a troll path, spawning enemies and hitting switches to shut gates before fighting him at the end. I wouldn't be surprised if the boss is actually a pvp thing online because he seemed pretty easy.
