I've had no issues whatsover but have seen an armored car drive downwards in to the road, do a front flip and then carry on as if nothing happened earlier.

Some people have been deleting the game app data and reinstalling to fix it. Just make sure to back up your save.

Was this the day one patch?
You could try uninstalling the game, unplugging your PS4 from the network and reinstalling from the disk so you're back on 1.00. From there see if the 1.00 version of the game loads your save data fine. Then reach a new checkpoint/area on that save file so it's re-written. After that try upgrading and reloading again. Wonder if it's a bug with reading the old save file location coordinates or something.
Obviously you should back up your saves before/after this.

pepper it with my launcher and then called in a bombardment, then got to the chopper and used the chopper's gun against it. It fell into a heap of dust then, and I thought it was going to jump out at me like Cloverfield but nope.

Good you got it sorted. With all the problems sounds like they didn't think people had saves already (as the game isn't released yet) and didn't test it properly with existing ones.

Did this actually happen? I remember there being talk about people worried about being banned, but I don't remember it actually happening to anyone. I used DSFix during all of my 300+ hours with that game on PC, constantly playing online with 60 fps on and I never had any issues.
