Realistically, I think the direction they have to go is darker. Make it feel more akin to the comics with Batman's super genius intellect and gadgets, but make the story and look of Gotham dark/gothic as all hell. I think a good template for that "feel" would actually be the concept art for The Joker:

Agreed, although it wouldn't be financially sane to go an even darker route.
 
I think think Riddler could have been in one of the films. Maybe just a smaller, Scarecrow like role...maybe a throwaway line about "Edward" helping the League of Shadows create confusion or encrypted data or something to that effect.

Joker would be in prison, like Hannibal Lecter in SOTL. Some new threat would be running wild and even Batman can't seem to stop him. Batman turns to the Joker for help in tracking the villain down, as only his twisted mind can see into the mind of the new threat. Joker helps, Batman gets baddie, but as in SOTL, Joker escapes at the end. Roll on Nolan's Batman 4, the 4th and final film, with Joker vs Bats endgame.

He did have a smaller role. He tried to blackmail Wayne Enterprises and almost revealed Batman's identity!

I like Joker as a Hannibal Lecter type background role, but if it panned out like that, it'd seem like too much of a ripoff of Silence of the Lambs. I think a smaller role for Joker than Lecter's would have worked wonders without feeling too derivative.
 
The only problem with Joker being in prison is that they would have made him take off his make up. He would have looked odd.

Well, naturally it wouldn't be a 100% SOTL ripoff, but they could steal liberal amounts of it and craft a very cool story for a Batman movie.

While this would have been nice to have (plus... y'know having Heath alive would be amazing), I'm glad that, creatively, the Joker didn't dominate this film series the way he could have. Bane takes this thing in a different direction.
