Equinox: Solstice II (Why can't this game get at least one little oddball GBA sequel at least!)
Battle of Olympus (It would be cool if they followed the tradition of the old game and put this on Wind Waker's engine, but Nintendo would probably never allow it.)
Granstream Saga (This was a great seller from what I heard, and saw on the actual cover! Why did this never get a sequel?)
Legend of Dragoon (Not dying for this, but would love to see it.)
Kingpin (More classic, "MOTHAFUCKAAAAAA, except please this time, actually have more black people in the game and let them actually be voiced by blacks, having you're dorm buddy impersonate blacks and hispanics is not fucking cool!"
Double Dragon (Seriously, this mechanic can totally work in 3d if done right.)
Real Leisure Suit Larry sequel ( seriously!
Mafia (I hope the sequel comes to light, despite all the inside rumors.)
Hexen! (Jesus Christ would this be a breath of fresh air for the fps(fpa?) genre, not to mention this game would look fucking beautiful on the Doom 3 engine!)
Last but hardly least ROAD RASH!!! (Where the hell is this game and why is EA just sitting on it's potential? A modern graphical overhaul along with the original games slick art style plus online play would be a dream come true. )

Punch Out!!, Mother/Earthbound, Wonderboy, Zombies Ate my Neighbors, Streets of Rage, Beyond Oasis, and Actraiser for sure. They are all awesome and long overdue for sequels.
Going with some more recent (but still 4+ years old) games, I'd like to see follow-ups to Blast Corps, Pilotwings 64, Jet Force Gemini, MDK 2, Toy Commander, Ecco the Dolphin, and Powerstone 2.
Looking through the lists and pics is giving me major nostalgia. So many great games.

Streets of Rage - The Beat 'em Up Genre has been lacking lately, with a severe lack of moves. We're talking about Knives that you can stab people with or throw, Baseball bats, pipes, and lots of other allsorted weapons.
Earthworm Jim - His variety of wacky projectiles would fit a Ratchet & Clank style game with more platforming elements.
Road Rash - Was fun back in the old days, but the Playstation sequals ruined it. The first PSX game was great though, the 3D0 one. Need more of that stuff.
Desert / Jungle / Urban Strike sequels. Soviet and Nuclear looked like crap, but I've never played them, how were they? The DS would be fantastic for the Strike series, since having the 2 maps would make the game work a lot better then pressing pause every 10 seconds.
Sparkster - Needs an Update like the second Megadrive game, loads of fun just boosting everywhere.
Ristar - One of my favourite Megadrive platformers.
Vectorman - Not that [now canned] sequal, and ACTUAL Vectorman game.
Mercs - I loved playing this Capcom game on the Megadrive. The Original Mode kicked the Arcade modes arse, and was just a heap of fun playing. Must get top down sequel!

I would love to see a real Landstalker (my favorite action RPG ever, beating out Zelda) sequel, but that's never going to happen. Alundra was good, but it wasn't the same, and let's not even discuss Timestalkers. I never did play Ladystalker, but don't remember hearing the best of things about it, and didn't like I saw emulated.
A new Monkey Island game from the original creator would also be the best game ever released.
