Are there any horror-themed role-playing games that are genuinely scary?

The primary examples that spring to mind without googling it are:

However, what these games have in common that they fall more into a category of action RPGs and immersive sims/first person shooters, which are genres that are generally easier to work with if you want to make a spooky horror game. What I'm wondering, however, is whether or not you could find a more traditional, horror-themed computer RPG, or even a jRPG game, which is designed around turn-based combat, party management, dialogue trees, dungeon crawling, and all that other stuff that you'd normally associate with games like Pillars of Eternity of classic Final Fantasy.

And if a "genuinely spooky game" with these criteria is too hard of a challenge to find, heck, let's even consider RPGs that simply have a very overt horror vibe to them. They should definitely be gory and dark in tone, ideally involving some demons, ghosts and ghouls, alien monsters, etc. But it can't be just any generic fantasy RPG because you could probably find all those things in most of them, but without those games being particularly focused on the horror aspect. It would have to be a game that wears its horror influences on its sleeve, kinda like The Secret World did (I know, it was an MMO but it's the closest example that I can think of right now).

And in case it's no obvious enough, some recommendations are mostly what I'm fishing for here.

Sounds like you want to play Shadow Hearts. All three games in the series are on the PS2. The first game is overtly gothic horror, the second game is similar but takes on a bit of a sillier tone. The third game isn't horror at all, so you could skip it. It's got turn based combat, party management, etc. Not exactly "genuinely scary", but I think it's about as spooky / scary as a turn based RPG can get.

There is also a prequel game for the PS1 called Koudelka, but it's... a bit rough around the edges.

Bloodborne comes to mind. It has horror elements to it but is more tense than terrifying.

Doesn't really fit some of your main criteria but if you never played it...

It sure checks a lot of boxes from the things you mentioned.

It is highly horror-themed, traditional in the sense that it brings you back to the roots of gaming, dungeon crawling in form of sinister chalice dungeons, party management through online "invasions", it runs deep in lore but less in the act of word, you take turns in battle :messenger_tears_of_joy: other wise you'd die in seconds.

I recently bought Darkwood for my Steam Deck whenever it arrives this month. I’m hoping it’s good.I am also replaying Parasite Eve and a lot of the mutations are creepy.

Sanitarium is a point and click adventure game, not a RPG type.

Ah yes, I guess most of Souls games could theoretically be classified as horror-themed, but Bloodborne definitely goes balls deep into that genre. It's not necessarily what I'm looking for because these games are almost exclusively focused on combat and exploration so that important aspect of having a party and dialogue system is missing.

Good recommendation, though. Love that game.

If you can keep an open mind over graphics and interface, then try Realms of the Haunting. A weird mix of point and click + FPS + horror adventure.


I don't mind the dated graphics, but I've seen a couple of reviews for this particular one and I was kinda put off by its more tedious aspects. And also how in typical 90s graphic adventure fashion, you can screw yourself over without even realizing it until it's too late. Not sure if I'd have the patience to deal with that bullshit, lol.

Parasite Eve is legitimately creepy.

Gameplay is sort of like Resident Evil meets Chrono Trigger.
I looked it up and the combat actually reminds me more of Vagrant Story. Interesting though, I'll put it on my list and give it a look.

Sanitarium is great. One of my favorite adventure games.

A lot of people are also mentioning Darkwood so I guess I'd better check that one out too.

Doesn't have the turn based combat but you do get to wander around finding stuff, talking to people and progressing the story. Original was an RPGMaker game that was actually quite good.


2D Action RPG in the vein of Silent Hill.

Haha it ain’t even 1/10th as deep as Vagrant Story so you can relax in that department. The horror atmosphere is what will keep you on edge. Many of Square’s games in that period had at least one obtuse gameplay mechanic (Vagrant Story, FFVIII, FF Tactics, Xenogears) but Parasite Eve is new player friendly :)
 
A horror themed RPG? Does that even exist anymore? Koudelka maybe? Parasite Eve? Shadow Hearts? All very old games, but I don’t think I’ve seen or played any modern games that fit that particular category.

That's just how classic computer RPGs were, hence my curiosity if any developer ever attempted to make a game like that. Lack of real-time combat doesn't mean it can't be scary. Being scary is more than just jump scares or tense combat situations. I'm more into games that instill a sense of foreboding with a dark, oppressive atmosphere and disturbing imagery. You can totally do that in a game that's not a typical 3D action game.

I wouldn't call it scary, but it's very unsettling. Very much like Silent Hill in that regard.
 
Darkwood was excellent, it was quite a nice surprise. That top down 2d game with its "crappy" graphics inspired more dread and tension than any other horror game I've ever played. Play it with headphones!

Perhaps. But no one on earth should be forced to endure the Sinking City. Best that game not even be mentioned so it can just be forgotten.

Idk why this game gets such a bad rap. It's like most people never actually bothered to play it and just decided that it's crap based off of some glitch compilation video that they saw on YT.

I mean sure, it's not some amazing triple-A baller but it's a perfectly enjoyable game and it's probably the only one of its kind (i.e. an open world RPG based on Lovecraft's stories). I certainly found it more memorable that Call of Cthulhu.

Its hard to say, many games fit that idea of horror, but what someone is "genuinely" scared about is just too subjective.

So I can recommend RE7, Evil Within 1 and 2 etc , but anyone can tell me they know its fake and to them not "scary".


So if someone wants to be really scared, drive the lower south side of Chicago really slowly for a few hours (if you make it to that long lol)

Roll down the window and say "O Block" or "600" really loudly, i hear its like bloody Mary to them if you say it like 5x lol

Darkwoord is insanely scary and has an amazingly twisted story. Don't let the simple overhead view fool you, the atmosphere and moment-to-moment terror is incredibly well done.

And RPG elements with skill unlocks through "XP".
