No worries, I've been playing the game without a guide and have been on a media blackout since TGS. Only asking other players if I get stuck.

There seem to be certain events that trigger what area you'll go to next, but you can always return to older areas. As far as bosses and enemies go, it's par for the course: You'll notice right away if you're in the wrong place

Very little of the story will be traditional narrative. Most of it is told in the space of mythology through item descriptions. There is also other subtle environmental and item interaction cues. The story is pieced together in time. You will get as much out of it as time you are willing to put in.

Completion time varies widely in Souls games. I got the Platinum in DS2 in less time than the average reported main campaign completion time at HowLongToBeat.com. Depends on numerous factors. With coop, it can be powered through reasonably fast.
