Dark Souls 3 Trailer (Gamescom 2015)

It isn't. The Dark Souls 2 pre-release demos had actually better assets and a improved lighting engine.

Looking at the completely real-time TGS 2013 trailer (which is 1080p, 60fps and must be running on PC) one can see not just the dynamic lighting & shadows but some of the better assets as well. More geometry detail in breakable things (the pillars), many more particle effects. I haven't see a good texture comparison but textures apparently took at hit also.

None of these things were present in the vanilla PC version (nevermind PS3, 360) nor any version of SotFS including PC ver.
 
It's obvious that this is the Bloodborne engine. Have we ever found out whether Bloodborne is/was using an upgraded version of the Demon's/Dark 1 engine? I'm not sure we've ever gotten confirmation that they were using the Phyrengine to begin with (as there was some conflicting info).

I'd love to know the the details behind the different iterations and I've never seen any concrete information.

I believe it's the Phyre Engine which they also used in Dark Souls, just heavily modified and upgraded. There are several similarities, like the way Bloodborne is culling things and how the animation of almost-out-of-screen monsters gets reduced to half the framerate as seen with the Hellkite Drake's tail in DaS1 and the Storm Beats in DeS, that could indicate it's still the same engine.
 
Downgrade talk is a little ridiculous considering nothing in the Dark Souls 3 showing looked out of the realm of Bloodborne.

All I want to know is what's the deal with the giant skeleton in the trailer.

I MUST KNOW.
