Does it ever wear out? I know you have to replace it if you change the heatsink, but what about besides that?

If you're not replacing the heatsink, you don't have to touch the thermal paste/grease.

It's kind of a urban legend, that the thermal grease eventually loses its effectiveness and you have to replace it. It's not true.
