I think there is a fair amount of issues - but I can't really tell if these issues are more frequent than any other device that gets used as frequently, and is so central to the day-to-day activity of the user. If anything, it could be that people just so badly WANT it to run smoothly.
Again, forums are the breeding ground of negative concerns. You won't get people chiming in and saying, "Ok my iphone was working well today. Check for another day!" (well not often).
My issues I've had with my iPhone:
Fingerprints
Possibly stuck home button
Coating wearing from the camera cover
Slowness
Apple Logo of Death
In accurate battery
Dodgy wi-fi reception
T-mobile shutting off access to the sidekick prepaid plan
Headphone remote not working
Spam callers
Removing headphone had iPhone still thinking they were plugged in
Push activation draining excessive battery
App crashes
Touch screen erraticness
Smugness
But really, a lot of these are not even hardware related, and often once-off issues that were corrected by changing my use. I mean, it was probably lint in the headphone socket causing the headphone issues - that would affect any device - how would you tell that it's an iPhone specific inadequacy?
The device is so amazing, you just WANT it to work perfectly. In that scenario, if not working perfectly all the time, and it being such a complicated device, it's hard to recommend, but that probably goes for any smart device. A lot of complicated things tend to be not 100% reliable.
The main example in my case is that I cannot rely on my iPhone ever rebooting properly (so I can't ever do the, 'oh just reboot your phone' thing). I need to be near my computer to fix it if it chooses to not reboot. And very few people are experiencing exactly the same thing as me, so I don't know how I can fix it. Fortunately, it seems to have gone away with the latest update, but I'm still a bit weary...
