It happens. If the ref knows it's not the finish, it usually ends with either a four count (ref hits the third count, counts one more in a panic) or the ref calls a kickout when one hasn't happened. Sometimes they use their fingers to indicate the wrestler just barely got his shoulder up.

I remember it happening with Rock vs Angle in one of their ppv matches I believe. Looked REAL bad.
Generally the ref would cover for something like that and imagine a phantom kickout or suddenly try to swat a fly in the middle of the count. Papa Shango was supposed to break up the Hogan vs Sid pin at Wrestlemania 8. He was like five minutes late, so Sid just casually kicked out of Hogan's "last" leg drop in the WWF instead, while Harvey Whippleman got disqualified by the ref for jumping up on the apron.
