A ton of things...
Scorpion, Kitana, Jade & Reptile added to the roster. Smoke is a normally selectable character. Ermac, Mileena and Classic Sub-Zero are unlockable via 3 UKKs. Human Smoke is playable after entering a button code while playing as Smoke.
5 new stages. Waterfront, Jade's Desert, Kahn's Kave, Scorpion's Lair and the hidden blue portal. The Bank stage from MK3 is gone.
New moves for characters, most notably Stryker's gun. Many tweaks to old characters. Sub-Zero's iceclone, Kabal's ground saw and Jax's missles weakened among other changes. Some combos do less damage.
You can now start chains with a jump punch/kick.
New difficulty tower and the return of Endurance Matches.
New, much harder(cheaper) AI.
Beating the game allows you to pick your reward from "Shao Kahn's Treasures". You can choose from up to 12 options, stuff like your regular story ending to a movie showing the various fatalities. The higher the difficulty you play the more of the 12 boxes you have access to.
2 on 2 and 8 person tournament modes added.
