Deffo Mounjaro or similar. I think you can drop weight way too fast with this stuff and it make you look sickly and too thin. Although he looks better here than previous pictures recently.

He has talked about the weight loss in some stream but I couldn't find it the last time this topic was up. Iirc he was cutting sugar among other things. Risk of diabetes perhaps?
Most people go fat when they get older, especially those with muscles in their younger years. Maybe he just want to catch the falling knife of aging before it's too late to not end up with sagging man boobs and a Studio Ghibli droop face?

Like I said earlier I think in some cases you just drop weight too fast leading to loose skin on the face and neck. He looks better than other recent piccy's, like his body has adjusted properly. Dudes dropped half his body weight in under 12 months* His body will definitely be thanking him though, way too many Dorito's and Mountain Dew before.
Hopefully this new found fitness enables him to keep saving gaming for longer.
