The reason it's in the funny pics thread is because neither event has happened.
I'm used to reading comics from right to left, so I keep reading it in the reverse order.
But evens till, I don't get the joke, or even what the "joke" is.

Uncle Sam has North America "on top". Hugo Chavez (the jacket is a dead give away, it's the Venezuelan flag) shows up and flips it upside down so South America is "on top". Doctors show up to inform Chavez he is dying of cancer. Sneaky ol' Uncle Sam decides to put North America back on top and smugly flips everything back again, but the Venezuelan (or the South American people in general) all rally together and flip it back as a team, knocking Uncle Sam on his ass.
