The only problem is the ending you will see is complete and utter ass. You don't have to. I know I don't. The game isn't about restrictions, but as much as possible letting you play how you want to play. Some quests can be finished in a simple conversation check. Some can be done by doing good things, and others by wicked, wicked things. For instance, in Rivet,there is a girl there who loves the Diego character (I think her name is something-Staley, anyways, she said that she needed some sort of ant-pheromone to get his attention, I gave her the goods, and nothing came of it. No quest, no xp, no nothing. I spent like 100 caps on that fucking shit and I got nothing. Also, there used to be an option to go up to the priest and tell him that Diego was sleeping her her, but the option has disappeared.

So I'll usually have him fix up all the hard to come by items like Power Armor; money spent on repairs can be gained back through trading. At the end you'll have tons of money anyway, so you're basically just looting valuables to pay for repairs (World of Warcraft-y!).
