Ralph was always a "special needs" student, intended to counter Lisa (a "gifted" student) and to show that the public school system was failing both of them.
He was eventually Flanderized. It's impossible to say when that happened, but season 9.
In the season 22 episode "Moms I'd Like to Forget", Chief Wiggum was shown (in a flashback) dropping Ralph on his head as an infant, giving him brain damage and causing immediate retardation.

By season 4 or something they made him a Wiggum and really dumb. And then they made him even dumber around season 10. And now they're smarting him up again to dumb with hidden depths like the president episode.
