The show shows a person looking through a sniper scope and the sound of a supposed gunshot mixing in with the sound of Ward releasing the cage. It's neither clear that it was Ward looking through the scope or that the gunshot actually happened. Though, the straightest way to theorize it is that Ward wouldn't be flashing back to things he didn't see, and the gunshot sound is objectively distinct.It was almost certainly Garrett who killed the dog after Ward failed to.
And now that dog will swim through heaven and hell to save Jemma and Leo.He didn't kill the dog because he was attached. It was a weakness then and he still has it. He hasn't been able to overcome that. I don't think he killed the dog or FitzSimmons.

It's people/things he's grown attached to. Garrett suggests as much multiple times. He shortly after Ward releases him, Garrett questions whether Ward is too close to the team, especially Skye. Garrett also sends Dethlok to tail Ward when he's with Skye to make sure that Ward follows through with Garrett's orders. Multiple times after Ward frees Garrett, Garrett refers to him as being weak.. just as he referred to Ward's attachment to the dog as a weakness.

If so, that would make Garrett pretty incompetent. He's got an agent working for him with a seeming inability to kill people he's close to. All Garrett would need to do is be a little more chummy with Ward and he'd be invincible.
