Yes you get the Scythe on that ending.

The 3 ending requirements are:

1) Talking to final boss, accept his offer.

2) Talk to final boss, reject his offer. You fight him and get the Scythe.

3) Before talking to final boss you eat all 3 Umbilical cords. Talk to final boss, reject his offer, beat him then there's another boss fight after it.
 
Took down the third boss finally! No blood vials or antidotes used in that fight. I think the secret was staying up in its grill the whole time and just rolling forward to dodge. Used the kirkhammer to do it

It gets a B at +10 for Arcane. Worth the points? I already hit soft cap for Skill which scales at A.

elevator next to the lamp has stopped working for me. The lever to summon it from above didn't work so I went around, but stepping on the button on the elevator doesn't do anything either. Is there any way of fixing this, or am I gonna be forced to take the long and annoying route to the boss each time? :(

Yeah you can get the scythe, what you do is there is 3 umbilical cords in the game, one from killing a story boss, one when you return to where to start the game in the clinic and one in the abandoned workshop.
Before you talk to Gehrman to end the game you eat all 3 of them, then refuse Gehrman, fight him and then the secret boss.
 
Is this true? I don't think it happened to me.

Gascoigne is found for the first time in Yharnam, at
the beginning of the bridge where the Cleric Beast is fought. If the player approaches him, he will help the player against a pack of Lycanthropes. If a player uses the coop bell before the Cleric Beast, he will help him as well.

Just finished.. some thoughts about the ending
- Gherman is the best fight in souls history. Just fucking awesome all around.
- Have to say, since after Rom, I completely lost track of the story.
I did the "true" ending, I have no idea what happened there at the end.
- Moon Presence is pretty cool, but they should have changed the skybox or something.

Onto my second play through! not sure if NG+ or a new one
 
Damnit, BS death. Word of warning for people who want to try this. From the Cathedral Ward lamp, when you eventually open up the door on the right, if the elevator isn't there, dont jump down in the hole. It looks like there is a normal floor under it, but you will just fall through it and die. I used my camera to look around and I could swear I saw a secret doorway under there with a full floor to land on.

Yes I now have some
Arcane spells
myself. About 3 of 'em actually. They sound awesome, so time to level that stat up when I've maxed out HP/STR.

Somebody probably has better advice I'm not all pro at this but it didnt take me too long.

For me the main thing was to stop trying to create distance. I only did this when I would need to heal up or use an antidote and in that case I'd run and use the pillars to create distance.

In battle I stayed really close and dodged towards it. This would put me directly behind it and I could get out 4 attacks or so. That's all it really took. You can really punish it close up.

I'm probably halfway through the game at this point but I'm curious what your general thoughts are without giving away any real spoilers. How do you like it compared to the others in the series? Is the second half of the game as good as the first?

Yep. I've got both the lamp > mill elevator and the mill > boss elevator unlocked, but the lamp > mill one just completely stopped working. It doesn't even do anything when I step on its button in the mill, where it'd normally go up to the lamp.

e: Sometimes the lever at the top will let me pull it instead of saying it's inoperable, but nothing happens.

Trying to avoid spoilers and figure everything out myself (this thread is a minefield! lol) but now I have to ask... at what point should I be able to equip runes?

Seems like I'm pretty far into the game not to be able to use them, I already have a bunch.
 
The first time I found Eileen the Hunter I thought she was an enemy and I attacked her and then she killed me. Now every time I go near her she attacks me. Is there any way for me to get her to talk to me anymore? I wanted to try out her quest line.

I did the same thing, lol. Looked like a clear secret but you fall right through the floor.

Be aggressive. Hold O while going up the ladder and keep moving to him when you reach the top. This ensures he stays near the edge of the tower.

Dodge roll through his first gun shot and you'll have closed the distance. Spam your gunshot. He'll be caught in a stunlock and suffer pushback. Repeatedly do this and he'll fall off the side of the tower.

When he dies, exit the game and reload, and the prize he drops will be by the gattling gun that was terrorizing you.

Ah okay. Knowing thats all it takes to get that
ending, makes things much easier for me. I was going out of way trying to save all the NPC's etc., but I can save that for NG+ playthrough on my 2nd character. Now I can rush through NG and get the Scythe on her.

Question about the cord in the Clinic, do you have to kill Iosefka for it to drop, and I hear she only drops it once the paleblood moon is out (after Rom)?
 
Got back to where father G was and helped
crow hunter beat other (tough mofo) hunter. i love how she spells "hunt/hunter"

Yeah i dunno, I think it requires a commitment to the game to really be able to progress decently. One of the big things that would effect you the most though is that there is no pause button so in the event you need to go handle the kids during a boss fight or something, well, you're gonna leave your character to die.
