Honestly, all things being equal, they're not THAT terrible, but you are CONSTANTLY at considerable disadvantage every time you encounter them. Enclosed spaces with problematic camera angles, narrow walkways, tiny islands in a poison swamp that you can't roll in, sometimes multiples, etc. They hit really goddamned hard, my SL 166 character with maxed stamina bar and fully upgraded shield is easily staggered if I block instead of dodge. Plus From loves to play tricks of perspective, so you see them from a distance and think they're normal, only to have them run at you SUPER FUCKING FAST and attempt to clobber you.
God I hate dealing with them. There's an island in the swamp with two or three, I have no idea what's on it because I can't roll in the swamp to dodge, and they rush you long before you reach land.

The "brothers" in The Void. But couldn't even fight them if I wanted to. Game was just too difficult and obtuse.

The deathclaws in fallout. Even when I was max level I avoided them because of all the horrors they gave me with panicked VATS encounters.
I wish I was afraid of spiders, because there's a lot of choices there. A friend bought skyrim day one and stopped playing as soon as they ran into giant ass spiders.

Took me ages to level up Blade of the Darkmoon just because i hated going through the Painted World to get souvenirs off these guys.
Definitely the creepiest enemy deign in any game ever. Why...why do their lower bodies have to be so smooth and voluptuous? Fucking abomination.
When I first played the first silent hill I could only handle playing it on god mode. But when this guy still killed me in god mode... I broke and didn't play for days.
Also when I first played the first Turok...
I remember when you first enter the area... you only see a huge dark open room... then suddenly a laser starts sweeping across the room and you hear a T-Rex roar... It took many years for me to return to that game.

Mirelurks in Fallout 3. I have a phobia of pretty much anything with mandibles and an exoskeleton, especially sea life, so a giant horseshoe crab is pretty much my worst nightmare. It doesn't help that they basically attack you by rubbing their junk all up in your face because of the janky combat.

You have no idea how scary those things are when they are right next to you in Wind Waker. I remember playing the game turning the camera to the right only to see those things about to chomp my ass while grinning like that.
I shudder to think of how it will be in the HD version.

Here's another one I thought of.
I have a fear of spiders, and I generally don't like spiders full stop, but I've learnt to deal with them in games to a degree, now Locoroco had big muja muja's which remind me too much of moving floating spiders which i had eventually learned to deal with... but then on the last boss it's some big ass muja muja that's travelling at the speed of light trying to eat me. Freaked me out for days before I re-gained the balls to take it on and complete the game.

Haha, I shit my pants when this thing came out of the elevator (http://www.youtube.com/watch?v=zVwjHcvJ0Yc). The creepy roar didn't help either.
