The headphone has clearly been surgically implanted into the guy's throat for some reason. It also appears to be directly over his voicebox.
We then pan down and see the "sacks" pulsating on his chest.
So, is whatever's playing through the headphone giving "energy' to the sacks?
Would certainly tie into the trailer's theme of language and power ;-)
Solid Snake is not in this trailer. He is not blonde. It's likely just some sort of cinematic thing and we're only seeing Liquid.

I really need ground zeroes to go cheaper during the Steam sale this week, I feel a bit behind. I also never played Peace Walker... I plan to play it soon though. Are the events in PW super important in GZ / PP?

Still, The Les Enfantes Terribles implies heavily. that it is in fact Solid and Liquid we're looking at.
That was a scrapped concept where Pliskin was meant to diguise himself by dyeing his hair blonde, but they just gave him his normal brown hair in the final game. And it's pretty clear that the strong lighting in the briefing room is the cause of his supposed blonde hair. As I said before, look at GZ, where everyone though BB had grey hair.

Even though this doesn't really count to most people, The Twin Snakes Briefing Videos were all ingame 3D and Snake had his regular brown hair but with a MGS2/3 Mullet. Kojima still had a part in that game even if he wasn't the director.

I think that was done on purpose as a visual callback to classic-style Big Boss. We never see his hair that colour again.

Just because we see both of them doesn't mean they both exist. It's likely just a visual trick or something.

I think BB voicing over "les enfaunt terribles" just as the camera pans out of both makes it really easy to associate Liquid and Solid in that scene.
