[Pokemon] Red and Blue? Is Leaf canon? Did Red pick Pikachu?

Her name isn't "Leaf," it's Green.

"Red, Green, Blue (revision to fix Green glitches), and Yellow." Which were used in the Manga to name the player (Red), the rival (Blue/Green in Japan IIRC), and Yellow (as the first female in the Manga IIRC). Since "Green" wasn't used in the Manga canon/story-reasons, they gave the female avatar for the player character in the Gen-3 remakes it.
 
Just how they continued the story of Black and White with B2/W2, they really should continues the original gen's story one day with....

I thought Green (who picked Squirtle) showed up before Yellow in the manga.

Thanks to ORAS's timey wimey ball there is no longer any way to stitch a whole timeline together. They are divergent and displaced with only cursory nods to each now.

Its' like Dictator, Boxer, & Claw in the Street Fighter community: the Blue/Green regional name swapping gets confusing, especially when scanlations will frequently go by the Japanese names. Leaf is a name that's easier to follow.

The manga's naming conventions don't have any bearing on Generation I titles or their Generation III remakes. Here is Bulbapedia's article for Leaf.

As far as the manga is concerned, you linked to Bulbapedia, which describes the character's origins.

The girl's name is Blue in Japan, based off the Pocket Monsters Red, Green, Blue, and Yellow games. She is based off the prototype female protagonist that never made it to the final cut.

She adopted the "Leaf" character outfit during the FireRed & LeafGreen story arc of the manga.

Outside of Japan, the games that were initially released were Pokémon Red and Blue.

So, when localizing the manga, they changed the girl's name to Green and gave the boy a name that people could correlate to a game that was actually released outside of Japan: Blue.

So effectively, the boy is known as Blue (Green in Japan), and the girl is known as Green (Blue in Japan).

Also, Yellow wasn't introduced until near the end of the first story arc.

In the Japanese version she's named Blue.
