Well not all skills, Big Boss speaks russian, while Venom couldn't and needed to fulton a translator for that. The question is: where is Big Boss all this time around? Building Outer Heaven using the ressources Venom collected?

It seems so. He does attack Venom, but in the meantime he also mops up all special forces. Its like hes part of the ruse.
Ocelot tells Venom not to take too long to get used to his new self, lol.

I think it was a horn, the Volgin hallucination created by Mantis has the same horn growing from his head too.

A decoy to allow BB to build Outer Heaven in total secrecy without SkullFace and Zero noticing it since all their eyes are focused on Mother Base Venom is building as a decoy and thus it can be sacrified if it gets raided.
Ishmael is a very trained person and saves Venom even after being attacked, and he has Kiefer's voice.
But if the guide talks about a parasite changing voices does that mean BB's voice was changed and hence the use of Kiefer's one and then when the parasite is cured BB can get back his David Hayter voice (just like we briefly heard his voice in the E3 2015 trailer when he said:" With every change I changed too." referring to his voice chnage and maybe his body change after Venom reacieving a surgical operation to look like Big Boss).

I think the face creation before the campaign starts is just some sort of a hint towards someone being reconstructed into BB.
To ensure his own safety? This Venom was probably the perfect candidate or something.
The whole world wants BB dead, so BB is unrecognizable. He literally puts this burden on Venom Snake, even tells him so. In the meantime he does his own shit.
But the forces find out whats happening at the hospital and take no prisoners. Mantis seems to be on BB's side, he annihilates the special forces and with his attacks on VS he constantly creates an escape route.

Remember real BB also took a hit. He probably had to recover also.
Why at the same hospital, beats me. Needed his DNA and stuff?
So here's a summary of some interesting things during the streams:
Story
+The scene where Big Boss puts down the cassette player is apparently at Outer Heaven (I didn't see this, but others are reporting it)
+You customize your face at the beginning of the game. There are a ton of options, and it is possibly the MGO character creator. Despite confirming your selection, you never actually get to use it.
+Quiet kills the nurse and doctor (who has a damn gun). She has BALLIN' black SWAT type gear.
+Quiet is about to kill you, but Ishmael comes in and saves you. Ishmael gets stabbed but it ain't no thing. He throws some liquid at Quiet, and she eventually catches on fire and falls out the window.
+There are soldiers in black killing anyone they find in the hospital. They wear helmets, but you can see inside one somewhat and he may be one of those Fog soldiers.
+Fire Volgin and Mantis show up and kill all the soldiers who are attacking the hospital. Fire Volgin is impervious to bullets, and can absorb energy and ammo to shoot back at enemies.
+In the lobby, Ishmael dives off the balcony and you shoot a couple of soldiers when Fire Volgin shows up. A tank crashes through and blasts Volgin, but he absorbs the shot.
+When you escape outside, Ishmael appears in an ambulance and tells you to get in. As he drives away, Fire Volgin attacks.
+Copters and other soldiers are firing at the ambulance. It eventually crashes and Ishmael either dies or is knocked unconscious.
+A copter appears above Venom Snake, but Mantis also pops in. A giant flaming whale attacks the copter.
+Ocelot comes in on a horse and tells you to hop on.
+Fire Volgin comes in with a flaming unicorn and you shoot it until lighting strikes a bridge and you collapse.
+You fall into a stream but recover. Ocelot leads you out of the area on a whaling boat.
+You ride from Cyprus to Afghanistan on the boat. It shows a montage where you suit up and get your new arm. It is worth noting that Big Boss still has his right eye.
+Your first mission is to save Kaz. Ocelot also has these ballin' shades.
Other
+Ishmael calls you Ahab and helps guide you out of the hospital. If you die he says, "Ahab? Ahab! AHAAAAAAAAAAAB!"
+Ishmael teaches you how to shoot and says "You're pretty good."
+If Ishmael dies, you get a Time Paradox. Keep in mind you also got this if Sokolov or Ocelot died in MGS 3.
+There is a poster of the Ground Zeroes promo art with "Kojima Productions" on it in the hospital.

From what I read and saw, I am almost sure this faithful lieutenant aka Venom Snake is Gray Fox just like the old speculations from previous years. Oceleot, In Afghanistan said to venom Snake: "You are a legend to the eyes of those who worship you. Let the legend live again!" which is clealry meaning that he is a decoy but will try to play the role of the legendary Big Boss, and only Gray Fox can go that far for Big Boss even after his death.
Getting Time Paradox when Ishmail dies just confirms that he is very important to the game eventhough you won't see him later which means he is Big Boss.

He meant when he gets shot from Fog Skulls bullets and dies through the gameplay, it just happens like in MGS3 which creates a Time Paradox. But he is meant to drive Venom Snake using the ambulance later.
I meant Ishmael doesn't die when he's driving the ambulance while he and Venom/Ahab are escaping ;-)

Yeah but do we make our MGO character THEN? That would be pretty strange. I'm thinking it's just to trick us initially, but that same character customization menu will be present in MGO.
And I meant if he dies when he's teaching you how to shoot, it did happen in one stream.
They are killing these streams so quickly, basically no one is getting past the hospital.

Ishmael can die if you don't shoot the guy on time. You will get a time paradox. Which I doubt is about Ishmael having to drive the ambulance later. You don't get a time paradox when Eva dies near the end of MGS3 either. I think?
Time paradox is about killing guys who already appeared in older 'future' games, so technically they can't die yet.
Seeing how Strangelove has been made Otacon's mom, I wouldn't be surprised if GF turns up just to go full circle in Hideo's ridiculous ways.
Medic = Gray Fox wouldn't surprise me in this franchise.

You mean like the Tobi isn't Obito from the detractors of that time and then look what happened at the end.
I'm pretty sure the avatar creation is just for FOB Motherbase invasion, but this hasn't been confirmed ;-)
