In the grimdark world of Dragon Age I figured elective surgeries like these wouldn't be a thing considering how it's such a struggle to stay alive and there are SOOO many things that will kill you.

No, those are regular scars to make your character look more badass, and those would fall under the basic 'Scars' category in the character creator.Maybe I'm misunderstanding by "surgery scar".

Nothing more immersive in a sword and sorcery high-fantasy RPG than references to modern-day cancer surgery procedures and F to M gender reassignment surgeries.I guess the positive previews - even from well regarded YouTubers were so galling that people had to make a big deal about this shit.
It's a toggle in the character creator that you very easily can avoid. None on that is forced on you, there's no glowing arrows forcing you to look at it. It's not mandatory by any means. And while that's likely trans coded, quite an appreciable number of women undergo mastectomy to fight breast cancer. A friend had one some years ago…sadly still passed away when the cancer came back.
