Crisis Core - Final Fantasy VII: An orchestra medley with all the tunes of the game. I liked it because it's a nice way to remember everything that happened throughout Zack's journey, specially since the majority of tunes are very memorable
I've always had a soft spot for Pokemon Gold/Silver's credits theme. It's so damn catchy. The remake's remix is also good, but it lacks the energy of the original.

Lightning Returns FFXIII - "Credits - Light Eternal" (has the motifs of music from all 3 games, really cool)

Lol yeah I remember the first time I beat this game, I must have had the volume up too high or something and the beginning bit scared the shit out of me.

I remember just putting down my Wiimote and feeling a teardrop roll down my face when this started playing. Game got me so good and the fact that the main melody is a remix of the game's Court theme just brought it home for me.

Undertale's credits theme is also a very good one that hasn't already been mentioned. So many cool variations on the themes you hear throughout the game.

Persona 3 had been my favorite credits theme for a very long time, but yeah, as soon as Last Goodbye kicked in at the end of the Undertale credits I knew P3 had been dethroned for me. I also love the final touch of looping Memory until you quit the game. (Even the frigging credits songs feel tied into the narrative agh this game)

The big-band style of this game's OST was simply amazing. It'll never hold a candle to Galaxy or 64's in terms of pure emotion but the credit music itself is up there with Nintendo's best.

My personal favorite ending theme is the series is without a doubt the one from Resident Evil: Code Veronica X
