Level up when you reach a new lantern/unlock a new shortcut. Or if your bloodstain is getting big use the homeward bone equivalent, bold hunters mask i think it's called

Fighting the boss doesn't trigger the doll, having one insight does. It just so happens that encountering the boss is probably the first way everyone gets their first insight. Those skull items (I forget the name right now) seem to increase overall insight and there are some hidden around.

Game is kicking my ass. I've restarted three times to try out different weapons, still haven't beaten the first boss even though between all three characters I've logged over 8 hours.

Frame rate could be better and the load times suck, but holy hell do I love this game. As a huge Souls fan my expectations were skyhigh, and so far they've been met

Hard but doable if you're talking about the guy with the axe. His attacks do high damage and have some tricky swing arcs, but he doesn't have absurd health. He's basically there to teach you how to fight a big foe and dodge their moves.

The guys with bricks are chumps.
