I don't know how to explain the frame rate going down after that patch, but are you using the d3d9.dll file from the NewVegasNexus forums? If not, it might be worth a shot. Using that (and that specifically, not just copying from Windows) made the biggest impact for me. If you've already done that, then yeah, I've got nothing. I'm running the game at on high @ 720p. When I dropped it down to medium, I didn't notice much of an improvement. My notebook isn't a beast desktop but it should be able to handle this much.
